The causes of gynecomastia are varied and include hormonal imbalances (elevated estrogen relative to testosterone), puberty-related hormonal fluctuations, obesity, certain medications (including some blood pressure drugs, antidepressants, and steroids), liver or kidney disease, thyroid disorders, marijuana use, and idiopathic causes. Research shows a strong correlation between body mass index and gynecomastia — men with a BMI over 25 have approximately an 80% prevalence of the condition. Distinguishing true gynecomastia (glandular tissue enlargement) from pseudogynecomastia (fat deposition alone) is essential, as the surgical approach differs.
Dr. Agrawal employs three surgical approaches depending on the severity and composition of the breast enlargement. For mild cases with primarily fatty tissue, liposuction alone may suffice. Moderate cases with a combination of fat and firm glandular tissue benefit from a combined approach using liposuction plus direct gland excision through a small periareolar incision. Extensive gynecomastia with significant skin excess may require additional skin removal. In all cases, the goal is a natural-appearing, flat chest with inconspicuous scarring.

Recovery & Aftercare
A compression garment is worn over the chest for 4 weeks following surgery to minimize swelling and support the new chest contour. Sutures are removed at approximately 7 days. Most patients experience mild to moderate soreness for the first few days. Return to desk work is typically possible within 3–5 days. Light exercise can be resumed at 2 weeks, with strenuous upper-body activities and heavy lifting restricted for 2–3 weeks. Swelling continues to diminish over 2–3 months, with final results visible at 3–6 months.
