Gynecomastia (Male Breast Reduction)

Gynecomastia — the enlargement of breast tissue in males — is a remarkably common condition, affecting an estimated 40–60% of men at some point in their lives. The condition can cause significant embarrassment, avoidance of activities that require removing one's shirt, and reduced self-confidence. At Amit Aesthetics in Aligarh, Dr. Amit Agrawal offers expert gynecomastia surgery to help men achieve a flatter, more masculine chest contour with minimal scarring.

The causes of gynecomastia are varied and include hormonal imbalances (elevated estrogen relative to testosterone), puberty-related hormonal fluctuations, obesity, certain medications (including some blood pressure drugs, antidepressants, and steroids), liver or kidney disease, thyroid disorders, marijuana use, and idiopathic causes. Research shows a strong correlation between body mass index and gynecomastia — men with a BMI over 25 have approximately an 80% prevalence of the condition. Distinguishing true gynecomastia (glandular tissue enlargement) from pseudogynecomastia (fat deposition alone) is essential, as the surgical approach differs.

Dr. Agrawal employs three surgical approaches depending on the severity and composition of the breast enlargement. For mild cases with primarily fatty tissue, liposuction alone may suffice. Moderate cases with a combination of fat and firm glandular tissue benefit from a combined approach using liposuction plus direct gland excision through a small periareolar incision. Extensive gynecomastia with significant skin excess may require additional skin removal. In all cases, the goal is a natural-appearing, flat chest with inconspicuous scarring.

Procedures & Techniques

  • Liposuction alone for mild fatty gynecomastia
  • Combined liposuction and gland excision for moderate cases
  • Extensive excision with skin removal for severe gynecomastia
  • Periareolar incision for gland removal
  • Chest contouring and sculpting

Recovery & Aftercare

A compression garment is worn over the chest for 4 weeks following surgery to minimize swelling and support the new chest contour. Sutures are removed at approximately 7 days. Most patients experience mild to moderate soreness for the first few days. Return to desk work is typically possible within 3–5 days. Light exercise can be resumed at 2 weeks, with strenuous upper-body activities and heavy lifting restricted for 2–3 weeks. Swelling continues to diminish over 2–3 months, with final results visible at 3–6 months.

Frequently Asked Questions

What causes gynecomastia?
Gynecomastia has multiple potential causes including hormonal imbalance, puberty, obesity (BMI over 25 correlates with 80% prevalence), medications (such as certain blood pressure drugs, antidepressants, and anabolic steroids), liver or kidney disease, thyroid conditions, marijuana use, and sometimes no identifiable cause (idiopathic). A thorough evaluation helps identify contributing factors.
Can gynecomastia be treated without surgery?
In some cases of recent-onset gynecomastia — particularly in adolescents — the condition may resolve on its own or with medical management (hormone therapy). However, long-standing gynecomastia with established glandular tissue and/or significant enlargement typically requires surgery for definitive correction.
Will gynecomastia surgery leave noticeable scars?
Scars from gynecomastia surgery are designed to be as inconspicuous as possible. Liposuction requires only tiny puncture incisions (3–4mm) that heal nearly invisibly. Gland excision is performed through a periareolar incision — placed along the border of the areola where the color transition naturally camouflages the scar. Even in extensive cases requiring skin removal, scars are positioned strategically to minimize visibility.
Can gynecomastia recur after surgery?
Recurrence after properly performed gynecomastia surgery is uncommon. If the glandular tissue is fully excised, it cannot regrow. However, significant weight gain, anabolic steroid use, or certain medications can potentially cause fat re-accumulation in the chest area. Maintaining a stable weight and healthy lifestyle helps ensure lasting results.
